Physical Therapist (PT) – Home Health

Full-Time | $75,000 – $85,000 Salary | 100% Employer-Paid Medical

We are seeking a compassionate and motivated Physical Therapist (PT) to join our growing Home Health team. This position offers autonomy, one-on-one patient care, and strong clinical support in a collaborative environment.

Compensation

• Salary: $75,000 – $85,000 annually

Responsibilities

• Conduct comprehensive patient evaluations in the home setting

• Develop and implement individualized treatment plans

• Provide therapeutic interventions to improve strength, mobility, balance, and safety

• Educate patients and caregivers on home exercise programs and fall prevention

• Coordinate care with physicians and interdisciplinary team members

• Complete accurate and timely documentation

• Ensure compliance with Medicare and home health regulations

Qualifications

• Active Physical Therapist (PT) license

• Home Health experience preferred but not required

• Strong clinical assessment skills

• Valid driver’s license and reliable transportation

• Ability to work independently in the field

• Excellent communication and organizational skills
