Home Health Physical Therapist – Multiple Locations Across Illinois

Compensation: Per-Visit (PPV) + $10,000 First-Year Success Bonus + $5,000 Sign-On Bonus
Schedule: Full-Time, Monday–Friday (Day Shift) + Weekend Rotation

About the Role

As a Physical Therapist on our client’s Home Health team, you’ll provide one-on-one care to patients in the comfort of their own homes. This role focuses on helping patients regain independence, improve mobility, and enhance their overall quality of life through customized therapy programs.

Key Responsibilities

• Perform comprehensive Physical Therapy assessments and develop individualized treatment plans
• Document and report patient progress and changes in condition
• Educate patients and caregivers on therapeutic exercises, equipment use, and ongoing care
• Supervise Physical Therapy Assistants and, when appropriate, Certified Nursing Assistants
• Maintain accurate records and participate in staff and agency development activities

Schedule & Travel

• Hours: 40 per week
• Schedule: Monday–Friday, day shift (hours vary by patient needs)
• Rotation: 1 weekend every 6 weeks (Saturday & Sunday day shift) + 2 holidays per year (1 summer, 1 winter)
• On-call: None
• Travel: Local travel to patient homes required

Qualifications

• Active Illinois Physical Therapy license
• Minimum 1 year of Physical Therapy experience (Home Health or Inpatient Rehab required — no new grads)
• Strong communication and documentation skills
