    Key Responsibilities

    • Conduct comprehensive physical therapy evaluations for adult patients in their homes using diagnostic muscle, nerve, joint, and functional ability tests and measures
    • Develop and implement individualized treatment plans following physician orders, addressing impairments and functional limitations with goals of maximizing patient independence
    • Perform periodic re-evaluations and modify care plans in consultation with physicians as patient needs evolve
    • Provide hands-on physical therapy treatments including therapeutic exercises, manual therapy, gait training, and mobility enhancement
    • Educate patients and caregivers on proper use of prosthetic devices, orthotic equipment, and durable medical equipment such as canes, walkers, and wheelchairs
    • Maintain accurate, timely clinical documentation in medical records on the date of each visit
    • Collaborate with interdisciplinary healthcare teams through case conferences and care plan development
    • Participate in physical therapy staff meetings, in-service presentations, and continuing education opportunities
    • Ensure appropriate safety practices and proper maintenance of all therapy equipment
    • Communicate effectively with patients, families, caregivers, and healthcare providers regarding treatment progress and goals

    Required Qualifications

    • Graduation from an accredited Physical Therapy program (DPT, MPT, or equivalent degree)
    • Current, valid Physical Therapy license issued by the New York State Department of Education
    • Minimum of one year of practical experience as a licensed physical therapist
    • Valid driver's license and reliable transportation for home visits throughout Franklin County
    • Physical ability to perform medium work: exerting up to 50 pounds of force occasionally to lift, carry, push, and pull objects including assisting patients with mobility
    • Ability to work in non-office, home-based settings with occasional exposure to outdoor conditions
    • Strong clinical assessment, critical thinking, and problem-solving skills
    • Excellent communication and interpersonal skills for patient education and team collaboration
    • Proficiency with electronic medical records and healthcare technology
