Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate and dedicated Physical Therapist for a PRN field position providing exceptional in-home therapy services throughout the Fort Worth area.

Responsibilities
Evaluate patients and provide skilled physical therapy services according to physician orders and individualized treatment plans
Implement therapeutic exercises and interventions to improve mobility, strength, balance, pain control, and functional independence
Assess patients’ home environments and recommend equipment and safety modifications to support successful recovery
Educate patients and caregivers on home exercise programs, mobility techniques, and fall-prevention strategies
Complete timely and accurate clinical documentation in the electronic medical record
Collaborate with the interdisciplinary team to coordinate patient care and communicate patient progress
Maintain compliance with agency policies, Medicare regulations, and all applicable state and federal standards
Travel to patients throughout Fort Worth and the surrounding area
Qualifications
Current Texas Physical Therapist license in good standing
Graduate of an accredited physical therapy program
Minimum of one year of physical therapy experience
Home health experience preferred
Current BLS certification
Valid Texas driver’s license
Reliable transportation and current automobile insurance
Strong organizational, communication, and clinical skills
Experience with electronic medical records preferred
