    Summary

    We are seeking a licensed Physical Therapist (PT) to join our interdisciplinary pain management team. This role is responsible for providing comprehensive physical therapy services to patients with chronic pain and functional impairments in an outpatient clinical setting.

    The Physical Therapist collaborates closely with providers, medical assistants, and behavioral health staff to design and implement individualized treatment plans focused on improving mobility, reducing pain, and restoring function.

    Candidates with experience in pain management, orthopedic rehabilitation, functional medicine, or integrated medical practices are strongly encouraged to apply.

    Responsibilities

    Perform comprehensive physical therapy evaluations and ongoing reassessments of patient functional status

    Develop and implement individualized treatment plans based on clinical findings and physician referrals

    Provide therapeutic exercises, manual therapy, and physical interventions to improve mobility and function

    Administer treatments including soft tissue mobilization, traction, ultrasound, and other approved modalities

    Instruct and motivate patients in therapeutic exercises and activities of daily living

    Educate patients and families on home exercise programs and injury prevention strategies

    Monitor patient response to treatment and adjust care plans as clinically appropriate

    Document all evaluations, treatments, and progress notes in the electronic medical record in accordance with clinical and regulatory standards

    Communicate assessment findings and progress updates to referring physicians and clinical staff

    Coordinate care with other members of the interdisciplinary healthcare team

    Provide clinical supervision and guidance to Physical Therapy Assistants when applicable

    Ensure all patient care is delivered safely, timely, and in alignment with documented treatment goals

    Required Qualifications

    Graduate of an accredited Physical Therapy program

    Active Physical Therapist license in the State of Illinois

    Current Physical Therapist certification

    Minimum of 2 years of clinical experience preferred

    Proficiency with EMR documentation

    Strong communication and patient education skills

    Ability to work effectively within an interdisciplinary medical team

    Preferred Qualifications

    Experience in pain management, orthopedics, or outpatient rehabilitation

    Experience working in an integrated medical practice setting

    Manual therapy certification or advanced musculoskeletal training

    Experience supervising Physical Therapy Assistants

    Familiarity with chronic pain patient populations
