Anova Care is actively seeking a compassionate and motivated Physical Therapist (PT) to join our expanding Home Health team. This is a great opportunity for clinicians who are passionate about providing personalized one-on-one patient care while enjoying flexibility and autonomy in their schedule.

As a Home Health PT, you will help patients improve mobility, regain independence, and enhance their quality of life in the comfort of their own homes.

Responsibilities:

Evaluate and assess patients’ physical therapy needs

Develop individualized treatment plans

Provide therapeutic exercises and rehabilitation services

Educate patients and families on treatment plans and mobility improvement

Coordinate care with physicians and interdisciplinary team members

Complete timely and accurate documentation

Qualifications:

Active Colorado Physical Therapist (PT) license

Home Health experience preferred, but not required

Strong communication and patient care skills

Reliable transportation preferred

Schedule: PRN / Flexible Schedule
