Physical Therapist – Full-Time
Occupational Health | QuadMed | Beaver Dam & Mayville, WI

Sign-On Bonus: $5,000 for full-time hires

Overview:
QuadMed is hiring a full-time Physical Therapist for occupational health at employer-based clinics in Beaver Dam and Mayville, WI. New grads are welcome; one year of experience preferred. Part-time candidates with 32 hours per week may also be considered.

Caseload:

• Treat patients of all ages, focusing on occupational health

• Mix of workers’ comp, industrial rehab, ergonomics, return-to-work, and general ortho cases

• Approx. 8–10 patients per day for full-time PTs

• Opportunities to develop specialties and earn certifications

Compensation & Benefits:

• $87,000–$110,000 per year, depending on experience

• 401(k), PTO, holidays, and other benefits

Schedule:

• Monday–Thursday (typical 8:00 AM – 4:30 PM)

• Flexible scheduling based on candidate availability

• No weekends or major holidays

Requirements:

• WI state licensure as a Physical Therapist

• Full-time, 40 hours/week preferred

• Candidates with 32 hours/week considered

• Legally eligible to work in the U.S.
