📝 Your Core Responsibilities:

Conduct comprehensive in-home physical or occupational therapy assessments

Develop and implement personalized care plans focused on mobility, safety, and activities of daily living (ADLs)

Provide hands-on therapeutic interventions and progress monitoring

Educate clients, families, and caregivers on safe transfer techniques and home modifications

Maintain clear, reliable communication with families and our clinical coordination team

✔️ What We Are Looking For:

Active Massachusetts Physical Therapist (PT) or Occupational Therapist (OT) license (Required)

A compassionate, professional, and dependable clinical work ethic

Experience in home health, rehabilitative centers, or assisted living environments is preferred

Strong communication, paperwork management, and teamwork skills

Reliable transportation to attend scheduled client visits on time
