About the Role:

AlwaysNear Home Care LLC is looking for a dedicated and compassionate Physical Therapist (PRN) to join our growing team in Atlanta, GA. Responsibilities:

Conduct comprehensive physical therapy evaluations and assessments for home care patients

Develop and implement individualized treatment plans to restore function and improve mobility

Provide therapeutic interventions including therapeutic exercise, manual therapy, and neuromuscular re-education

Monitor and document patient progress, adjusting care plans as clinically appropriate

Educate patients and caregivers on home exercise programs and injury prevention strategies

Collaborate with interdisciplinary home health team members to ensure coordinated patient care

Maintain accurate and timely clinical documentation in compliance with regulatory standards

Requirements:

Valid Georgia Physical Therapist (PT) license in good standing

Graduate of an accredited Physical Therapy program (DPT or equivalent)

Home health or outpatient physical therapy experience preferred

Excellent communication, interpersonal, and patient education skills

Reliable transportation and valid driver's license for home visit travel throughout Atlanta, GA

Ability to work independently and manage a flexible PRN schedule
