Evaluate patients’ physical abilities and develop individualized treatment plans.
Implement therapy programs focused on strength, balance, and mobility improvement.
Educate patients and caregivers to promote independence and safe recovery.
Collaborate with nurses, therapists, and physicians to ensure coordinated, holistic care.
Maintain thorough and timely clinical documentation for all patient interactions.
Active Physical Therapist (PT) license in the State of Arizona.
Current CPR certification and valid driver’s license with reliable transportation.
Strong communication, assessment, and critical-thinking skills.
Prior home health experience preferred — new graduates with passion and initiative are encouraged to apply!
