Job Summary:
Our Client is seeking a skilled and compassionate Physical Therapist (PT) to join thier Home Health team in Corona, CA. As a Physical Therapist, you will work with patients in their homes to assess, plan, and implement rehabilitation programs that promote optimal physical function, mobility, and pain management. Thier goal is to help patients achieve their highest level of independence and quality of life.

Requirements:
\- Current Physical Therapy license in the state of California
\- 1-2 years of experience in home health physical therapy, preferably with geriatric patients
\- Strong clinical skills in assessment, treatment planning, and implementation of physical therapy interventions
\- Excellent communication and interpersonal skills
\- Ability to work independently and as part of an interdisciplinary team
\- Reliable transportation and valid driver's license

Preferred Qualifications:
\- Experience with electronic medical records (EMRs)
\- Certification in specialized areas such as geriatric physical therapy or wound care
\- Bilingual or multilingual skills
