What You’ll Do:

• Design and implement personalized care plans for each patient

• Deliver hands-on therapy that helps patients regain strength and confidence

• Educate families on safe mobility, exercises, and home care practices

• Work closely with a collaborative care team to ensure consistent, quality outcomes

• Keep accurate, timely clinical documentation

What We’re Looking For:

• Licensed Physical Therapist (PT) in Arizona

• Home health experience is a plus—not a must

• Strong communication and people skills

• A heart for helping people regain independence
