Job Description

We are looking for a licensed Physical Therapist (PT) to provide home health visits to patients in their homes. The PT will evaluate patients, develop care plans, and provide therapy to improve mobility and function.

Responsibilities

Perform patient evaluations

Provide physical therapy treatments

Develop and follow care plans

Educate patients and caregivers

Complete documentation on time

Requirements

Active California PT License

Home health experience preferred

Valid driver’s license and reliable transportation

CPR certification
