    What you'll do

    • Deliver skilled PT visits in patient homes

    • Build and update care plans with fast approvals

    • Document quickly using modern EMR (no outdated tools)

    • Track outcomes that matter—mobility, independence, fewer readmissions

    • Partner with nurses, aides, and families for full care support

    • Take ownership of your caseload, day one

    Must-haves

    • Active PT license in Missouri

    • 1+ year home health or outpatient PT

    • Valid driver's license and reliable car

    • Able to manage 5–6 visits/day

    • Work authorized in US
