This position requires a valid driver’s license, proof of insurance, and a reliable personal automobile for local travel to provide care to clients in the Collin County, Texas area. Mileage is reimbursed for start of care and regular visits.

Responsibilities: ● Provide direct patient care to a diverse range of patients ● Complete start of care and conduct thorough evaluations of patients to identify physical impairments and develop personalized treatment plans. ● Utilize a range of evidence-based techniques, including trigger point therapy, to address patient needs. ● Implement and monitor patient treatment plans, adjusting as necessary. ● Collaborate with healthcare professionals, including physicians and occupational therapists, to ensure comprehensive patient care. ● Stay current with the latest developments in physical therapy, including orthopedics, hospital medicine, and anatomy knowledge. ● Document patient progress and maintain accurate medical records. ● Educate patients on proper exercise techniques and provide guidance on maintaining a healthy lifestyle. ● Collaborate with other healthcare professionals to develop and implement individualized care plans ● Provide emotional support and educate patients and their families on disease processes and treatment options ● Assist in the development and implementation of policies and procedures related to patient care ● Participate in quality improvement initiatives and contribute to the continuous improvement of patient care
Qualifications: ● Bachelor's degree in Physical Therapy or Doctor of Physical Therapy (DPT) degree. ● Current licensure as a Physical Therapist in the State of Texas ● Strong knowledge of medical terminology and anatomy ● Minimum one year experience in a home healthcare setting ● One-year experience in geriatric care, preferred ● Experience working in a hospital or clinic setting. ● Ability to provide patient care in a variety of settings, including home community care. ● Excellent communication and interpersonal skills. ● Proficiency in trigger point therapy and other physical therapy modalities. ● A passion for delivering high-quality patient care and a commitment to ongoing professional development.
