    Job Summary The PT/PTA provides physical therapy services to help patients improve mobility, strength, balance, function, and independence and increase their quality of life. This position involves working with patients and families in the home, following an individualized plan of care, monitoring patient progress, and collaborating with the healthcare team.

    Essential Duties & Responsibilities (PT):

    Provide skilled physical therapy services in accordance with the patient's plan of care and physician orders.

    Conduct evaluations and assessments of mobility, strength, range of motion, balance, and functional abilities.

    Develop, implement, and modify individualized treatment plans based on patient needs and progress.

    Provide therapeutic interventions to improve strength, mobility, balance, coordination, and overall function.

    Monitor patient progress and response to treatment and re-evaluate as needed to optimize outcomes.

    Educate patients, caregivers, and family members on exercises, proper body mechanics, mobility techniques, home programs, and safety.

    Review medical records and collaborate with physicians, caregivers, and other members of the healthcare team.

    Accurately document evaluations, interventions, progress, goals, and other services provided.

    Provide guidance and support to other team members involved in patient care as appropriate.

    Follow infection control, safety, and universal precaution protocols.

    Participate in continuing education and maintain compliance with applicable regulations and payer requirements.

    Perform other duties as assigned.

    Essential Duties & Responsibilities (PTA):

    Provide physical therapy services under the direction and supervision of a licensed Physical Therapist and in accordance with the patient's plan of care.

    Implement therapeutic exercises and interventions established by the supervising Physical Therapist.

    Instruct, assist, and motivate patients during exercises and activities while ensuring patient safety.

    Monitor and document patient progress and response to treatment and communicate findings to the supervising Physical Therapist.

    Educate patients, caregivers, and family members on exercises, proper body mechanics, mobility techniques, home programs, and safety.

    Assist patients with the proper use of supportive devices, braces, prostheses, and other assistive equipment.

    Accurately complete visit documentation, progress notes, and clinical notes in a timely manner.

    Communicate effectively with patients, caregivers, the supervising Physical Therapist, and other members of the healthcare team.

    Follow infection control, safety, and patient care protocols.

    Participate in continuing education and maintain compliance with applicable regulations and payer requirements.

    Perform other duties as assigned.

    Qualifications: Physical Therapist (PT):

    Current and valid Texas State License as a Physical Therapist

    Current CPR Certification

    Previous experience as a Physical Therapist in an acute care, rehabilitation, or home health setting preferred

    Driver’s License (required)

    Car Insurance (required)

    Physical Therapist Assistant (PTA):

    Current and valid Texas State License as a Physical Therapist Assistant

    Current CPR Certification

    Recent work experience in Home Health (preferred)

    Driver’s License (required)

    Car Insurance (required)

    Skills:

    Strong interpersonal skills and good judgment

    Good listening skills; strong verbal communication skills with a diverse range of individuals

    Must have the ability to work well under pressure and stay calm and focused in emergent situations

    Be highly self-motivated and results-driven

    A desire for further education to keep up with the ever changing medical advances
