Physical Therapist Sheboygan Senior Community – Sheboygan, WI Pay Rate: $52–$55 per hour Schedule: PRN | Approximately 6–10 hours per week

Sheboygan Senior Community (SSC) has been proudly serving seniors in Sheboygan County for over 60 years. We provide a full continuum of care—including assisted living, skilled nursing, short-term rehabilitation, respite care, and end-of-life services. Our resident-centered philosophy and dedicated therapy team ensure that each individual receives compassionate, high-quality care.

We are currently seeking a Physical Therapist to join our therapy department. This position offers flexibility, a supportive work environment, and the opportunity to make meaningful impacts on the lives of our residents.

Responsibilities

Provide high-quality physical therapy evaluations and treatments to residents in skilled nursing and rehabilitation settings

Develop individualized treatment plans to support mobility, function, and overall well-being

Collaborate with an interdisciplinary team including nurses, occupational therapists, speech therapists, and other care staff

Document services in accordance with regulatory and facility standards

Educate residents and families on therapy goals, progress, and home exercise programs

Maintain professionalism, empathy, and a resident-first approach at all times

Qualifications

Current Wisconsin Physical Therapist (PT) license

Graduate of an accredited Physical Therapy program

Previous experience in a skilled nursing or rehab setting preferred, but not required

Strong communication, time management, and clinical skills
