A skilled nursing and rehabilitation facility in Hornell, NY is looking for a licensed Physical Therapist to join their team on a full-time, direct-hire basis. In this role, you'll evaluate and treat residents, build individualized rehabilitation plans, and directly improve mobility, independence, and quality of life. Your Day-to-Day

• Evaluate each resident's condition and design individualized physical rehabilitation plans tailored to their recovery goals
• Treat pain and restore function through therapeutic exercise, manual therapy, stretching, and physical therapy devices
• Guide residents through therapy tasks, track progress, and adjust treatment plans as needed
• Educate residents on what to expect at each stage of recovery to support confidence and engagement
• Supervise physical therapy assistants as required
• Collaborate with the interdisciplinary care team to ensure comprehensive, coordinated resident care
• Maintain a safe, clean treatment environment in compliance with infection-control policies

What We're Looking For

• Graduate of an accredited Physical Therapy program
• Current New York State PT license
• Experience in skilled nursing or long-term care settings preferred
