Job description:
Essential Duties and Responsibilities
Supports and promotes the mission and values of our organization.
Performs evaluative and therapeutic procedures under the order of a physician or other accepted healthcare provider (mandatory supervision requirements) while observing precautions, indications, and contraindications.
Evaluates residents per department protocols and establishes a therapeutic plan of care which includes functional long- and short-term goals, frequency and duration of services, and therapeutic modalities to be used.
Seeks input from the resident and primary caregivers regarding the therapeutic plan of care. Incorporates this information into the therapy goals.
Provides consultation to the referring physician and other members of the healthcare team regarding the plan of care and discharge needs of the resident. Attends all pertinent team meetings.
Monitors and evaluates the treatment effect and progresses the treatment program toward the functional goals established in the treatment plan of care. Modifies the treatment plan of care as necessary.
Documents in the Electronic Medical Record all evaluative and therapeutic interactions with the resident per department procedures. Ensures the documentation is accessible to other members of the care team and uses easy-to-understand language.
Recommends and obtains appropriate assistive devices for the resident, ensuring that instruction has been given in their use.
Provide appropriate education to the resident and/or caregiver. Documents the results of teaching.
Refers residents appropriately to the Physical Therapist Assistant. Works collaboratively with other therapy professionals to ensure high-quality resident care.
Understands and complies with Medicare and other reimbursement regulations.
Documents resident care appropriately with attention to Medicare, Medicaid, Private Pay, and other reimbursement requirements.
Provides in-service education regarding clinical knowledge and skills to nursing and other departments and maintains current clinical skills and knowledge through learning activities.
Actively assists with the development of new programs and services offered by Rehabilitation.
Maintains awareness of professional issues through membership in professional organizations and reading professional publications. Bring pertinent matters to the attention of other staff members.
Supervises the activities of the assistant-level staff when appropriate, considering their level of education and training. Ensures adherence to all state mandates and to professional organizations' standards for the use of assistant-level staff. Bring performance issues to the Director's attention.
Provides mentorship to new staff during orientation, ensuring that the department orientation checklist and basic competencies are completed per departmental procedure.
Ensures department cleanliness and maintenance by routinely keeping office and clinical areas clean and neat. Assists with routine cleaning of equipment and space. Follows all system and departmental infection-control procedures.
Maintains required licenses, certifications, and competencies, and completes annual compliance courses on time.

Education and Experience
Bachelor’s degree required. Master's or Doctorate preferred.
Active license as a PT in the applicable State of work.
Basic Life Support (BLS) certification is preferred
Prior experience in a long-term care or skilled nursing facility preferred
An equivalent combination of education and experience that provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.
