Perform in-home PT evaluations and treatments
Build and adjust care plans per MD and RN orders
Educate patients and caregivers on mobility and safety
Track and document progress in WellSky/Kinnser
Coordinate closely with RNs, OTs, STs, and case managers
Manage your own daily visit schedule with minimal office calls
Active Virginia PT license (in good standing)
Reliable transportation for field visits
EMR documentation experience (WellSky/Kinnser preferred)
Eligible to work in the U.S.
Ability to meet Medicare home health documentation standards
Pay: $90K–$105K base + productivity bonus potential
401(k):
PTO: 15 days + 6 paid holidays
Allowances: $30/month cell + $20/month internet
Team: 10+ PT/PTAs + full interdisciplinary support
Full-time, Monday–Friday
Field role covering Amelia, Powhatan, and Tri-Cities
Limited local travel within territory
