You'll help patients regain strength and mobility where it matters most—their own homes.
Perform in-home PT evaluations and treatments
Develop and adjust care plans per physician and RN orders
Educate patients and caregivers on safety and fall prevention
Track progress and document in EMR (WellSky/Kinnser)
Communicate plan changes to care team and providers
Coordinate closely with RNs, OTs, STs, and case managers
Active Virginia PT license (in good standing)
Reliable transportation for field visits
EMR documentation experience
Eligible to work in the U.S.
Ability to meet Medicare home health documentation standards
Pay: $100K–$135K annually (based on experience)
