Position Summary: We are seeking a reliable and experienced Physical Therapy Assistant (PTA) to join our team on a per diem basis. The PTA will provide rehabilitative services to patients in their homes under the direction and supervision of a licensed Physical Therapist. This role is ideal for a self-motivated professional who enjoys flexible scheduling and working independently in the field.

Primary Responsibilities:

Provide physical therapy treatments to patients in accordance with the Plan of Care established by the supervising PT

Monitor and document patient progress; report changes to the supervising PT

Instruct patients and caregivers on home exercise programs and safety

Collaborate with interdisciplinary team members to ensure coordinated care

Maintain timely and accurate clinical documentation in the EMR

Comply with Medicare, Medicaid, and agency standards and regulations

Qualifications:

Active Florida license as a Physical Therapist Assistant (PTA)

Minimum of 1 year of experience in home health or outpatient physical therapy preferred

Familiarity with Medicare documentation requirements

Reliable transportation, valid driver’s license, and current auto insurance

Ability to work independently and manage time effectively

Strong interpersonal and communication skills
