What You'll Do
Under the supervision of a licensed Physical Therapist, you will:
Implement individualized treatment plans designed to improve strength, mobility, balance, and function.
Guide patients through prescribed therapeutic exercises and activities.
Assist with gait and mobility training using walkers, crutches, braces, and other assistive devices.
Provide therapeutic modalities as prescribed.
Monitor patient progress and communicate significant changes to the supervising PT.
Educate patients and caregivers on home exercise programs and strategies to promote continued progress.
Assist patients with safe transfers and mobility.
Complete accurate and timely clinical documentation.
Participate in quality assurance activities, team collaboration, and ongoing training
What We're Looking For
Current Physical Therapist Assistant (PTA) license in the state of practice.
Graduate of an accredited PTA program.
Minimum 2 years of PTA experience preferred; home health experience is a plus.
Current CPR certification.
Strong communication and interpersonal skills
Ability to work independently and manage visits effectively.
Reliable transportation and willingness to travel to patient homes.
Ability to safely assist with patient transfers and mobility, including patients weighing 150+ lbs.
Ability to meet all applicable state and company compliance requirements
