JNOW HIRING a Physical Therapy Assistant for per diem OR part-time opportunities in the home care setting. You will be paid for each completed patient visit (with documentation).

WORK INDEPENDENTLY * 1-to-1 IN-HOME PATIENT CARE

RESPONSIBILITIES:

Perform home visits for patients in a home care setting

Under the supervision of the Physical Therapist, performs treatment modalities as indicated according to the plan of care

Implements plan of care to provide safe, efficient quality care to patients on a daily basis, reporting any changes to the Physical Therapist or Clinical Manager.

QUALIFICATIONS:

Current license as a Physical Therapy Assistant in Connecticut

Valid driver’s license

Reliable transportation and willing to travel in New London and/or Windham and/or Middlesex Counties

Strong communication and interpersonal skills

Home health experience including Medicare preferred

Graduate of a physical therapy school approved by: Commission on Accreditation in Physical Therapy Education-accredited physical therapist assistant education program and certified in Connecticut.
