Role Description:
This is a part-time home care role for a Physical Therapist at Hometown Fitness & Wellness, LLC. The Physical Therapist will be responsible for providing patient care and habilitation services. This includes creating personalized treatment plans, evaluating patients' conditions, and monitoring their progress. The Physical Therapist will work closely with individuals in their homes to help improve their balance, flexibility, coordination, and overall physical fitness. The Physical Therapist has complete control over scheduling and treatment modalities of doctor prescribed therapy.
Responsibilities:
Conduct comprehensive assessments of patients' physical conditions and develop individualized care plans.
Implement therapeutic interventions to enhance mobility and manage pain effectively.
Educate patients and their families on treatment plans, exercises, and home care strategies.
Qualifications:
Licensed Physical Therapist.
Two years experience in providing patient care and home care is preferred.
Excellent communication and interpersonal skills.
Ability to work independently.
Experience with individuals with intellectual and/or developmental disabilities is a plus.
Bachelor's or Master's degree in Physical Therapy or related field.
Pay predicated on experience.
