Responsibilities:

Perform comprehensive patient assessments and develop individualized care plans.

Deliver evidence-based therapeutic interventions to improve mobility and function.

Track patient progress and adjust treatment plans as needed.

Collaborate with interdisciplinary healthcare teams to ensure holistic care.

Educate patients and caregivers on therapy goals and home exercise programs.

Maintain accurate and timely clinical documentation.

Participate in continuing education and professional development activities.

Requirements:

Active Florida Physical Therapist License (Required)

Experience in home health or outpatient settings preferred

Strong interpersonal and communication skills

Self-motivated and able to work independently

Passion for delivering high-quality, compassionate care

Flexibility to work PRN hours as needed.
