Job Summary:
As a Direct Primary Care+ Physician Assistant at Exponential Health, you will provide comprehensive primary care services to a defined patient panel, focusing on prevention, chronic disease management, acute care, and health promotion. You will develop long-term relationships with patients while delivering accessible, evidence-based care that improves health outcomes and reduces unnecessary healthcare utilization.
The Physician Assistant will serve as a trusted primary care provider, managing a broad spectrum of health concerns, coordinating care across the healthcare continuum, and supporting employer-sponsored population health initiatives. This role emphasizes timely access, continuity of care, patient engagement, and proactive management of chronic conditions to improve overall workforce health and productivity.
Duties/Responsibilities:
- Medical Expertise: Provide preventive, acute, and chronic disease management services across the lifespan. Conduct comprehensive assessments, diagnose illnesses, prescribe treatments, and develop individualized care plans.
- Holistic Health Management: Utilize evidence-based preventive care strategies, risk stratification, and chronic disease management programs to improve long-term health outcomes and reduce avoidable healthcare costs.
- Member Connection: Provide personable, high-touch care that fosters rapport with up to 700 Members/Patients. Build trust through active listening, respect, curiosity, and self-awareness, ensuring a comfortable healthcare experience that enhances member satisfaction.
- Case Management: Maintain detailed and accurate medical documentation, updating charts by the end of the day. Schedule and prioritize tasks effectively to meet the diverse needs of Members. Monitor and manage patients with chronic conditions including hypertension, diabetes, obesity, and behavioral health concerns through ongoing follow-up and care coordination.
- Collaboration: Work closely with physicians, nurses, and specialists to ensure seamless coordination of care, enhancing overall member satisfaction. Collaborate with employers, care navigators, specialists, and ancillary healthcare providers to ensure coordinated, cost-effective care and successful patient outcomes.
- Communication: Provide timely communication through phone, secure messaging, telehealth, and in-person visits to ensure accessible care and minimize unnecessary urgent care and emergency department utilization.
- Business Acumen: Analyze current service offerings and identify opportunities for expansion and improvement in service delivery.
- Population Health Management:
- Identify high-risk patients and implement proactive interventions to improve outcomes.
- Utilize employer health data and risk assessments to develop targeted wellness and disease management initiatives.
- Monitor quality metrics and patient engagement measures to support organizational health goals.
- Represent the organization at employer and community-based events and activities.
- Support outreach and engagement initiatives with organizational partners.
Required Skills/Abilities:
- Proficiency with EMR software is required, experience with Epic is a plus
- Excellent interpersonal and communication skills, with a commitment to providing exceptional patient-centered care.
- Strong organizational skills with the ability to manage a diverse panel of patients effectively.
- Passion for innovative healthcare solutions and a proactive approach to patient wellness.
- Willingness to travel between office locations in Grand Rapids and Holland as needed to provide consistent and high-quality care.
- Strong understanding of primary care practice management and population health principles.
- Experience managing chronic diseases and preventive health initiatives.
- Ability to independently manage a broad range of primary care conditions.
- Comfortable utilizing telehealth and digital communication platforms to improve access to care.
- Commitment to improving healthcare quality while reducing unnecessary healthcare spending.
Education and Experience:
- 5+ years of experience in clinical care (required)
- Experience in family medicine, internal medicine, occupational health, employer-sponsored health clinics, direct primary care, or value-based care models preferred.
- Master's degree from an accredited PA program (required)
- PA Certification (PA-C) (required)
- Licensed from the Michigan Medical Board (required)
Physical Requirements:
- Ability to sit or stand for prolonged periods of time
