Your Role
In this role, you will deliver comprehensive primary care, partner with the Interdisciplinary Team (IDT), and help develop best practices to support high-quality outcomes.
Key responsibilities include:
Deliver comprehensive primary care and evaluate acute illnesses
Conduct in-person assessments and develop care plans with the IDT
Collaborate with Community-Based Primary Care Physicians and the IDT to ensure coordinated care
Supervise 2–3 Nurse Practitioners, supporting and overseeing complex cases
Perform and coordinate discharge planning for patients in acute or long-term settings
Participate in call coverage as part of a rotating schedule
Review outcomes data, participate in quality improvement initiatives, and develop best practices
Maintain a high level of documentation (Athena experience strongly preferred) to support care planning
How You Qualify
Completion of an accredited residency program (Internal Medicine, Family Practice, Emergency Medicine, or Geriatrics preferred)
Board Certified with an active California physician license
DEA Registration and State CDS Certification preferred
Experience with Medicare and Medi-Cal preferred
Minimum 2–3 years of experience, ideally in value-based care, primary care, PACE, geriatrics, or SNF settings
