     Outreach Coordinator

     

    Description:

    This position is responsible for the research, analysis, and organization of information about social services in the PA 211 East 7-county service area. Information is provided in multiple formats and for a variety of customers. Position interacts daily with helpline staff and social service professionals at local agencies. Clearances Required: At point of hire, and periodically thereafter, must secure the Pennsylvania State Police Background, Pennsylvania Child Abuse History. This is required for all positions in PA 211 East in accordance with contracts.

    In addition, this position will be responsible for answering incoming calls, collecting client information and demographics and providing appropriate referrals to clients seeking health and human services.

    Skills:

    Excellent writing and communication skills required. Must be detailed oriented and have a passion for improving access to information. Knowledge of social service delivery system strongly preferred.

    Accountability, Director, PA 211 East

    Essential Functions: Under the supervision of the 211 Director, the Community Outreach Specialist will:

    • Establish a deep understanding and familiarity of all 211 operations, services and key data points to effectively educate and convey our purpose and impact regionally.

    • Attend, participate, and present at regional organizations and community events such as health and school fairs, community/agency-sponsored events, etc., that provide opportunities for marketing 211 to individuals who could use our services, as well as discovering new programs that are available to assist them.

    • Professionally represent 211 as needed at events.

    Resource Navigator Duties to include:

    • Assess client need, current situation and evaluate client eligibility for various programs and specialized contracts and make referrals to appropriate service providers (via phone, text, email, chat and other medias).

    • Collect client information and demographics.

    • Interview callers to identify problems and assess needs.

    • Provide information on health and human resources for all counties in East region.

    • Refer callers to appropriate services.

    • Follow up on referrals when necessary; provide additional referrals and assistance when needed. • Maintain adequate client records to prepare monthly reports and others as assigned.

    • Demonstrated knowledge of Microsoft Office basics (Excel, Word, Outlook) and effective use of technology including multiple software applications, text, chat and headset.

    • Knowledge of social services is desirable.

    • Ability to make connections and network to provide awareness of services in the community..

    • Ability to communicate fluently -Bilingual Spanish/English a plus.

    Travel Requirement:

    To fulfill the responsibilities of the position, travel to various events, companies and/or agencies is required. Employee must have valid driver’s license and access to a reliable motor vehicle.
