    Job Description

    AHRC New York City is seeking a Placement Specialist. Under the direction of the Assistant Director of Intake & Enrollment, the Placement Specialist will be responsible for coordinating the enrollment procedure for individual people identified to receive selected services through AHRC NYC. The focus of the position is to promote an efficient entry into services for individuals identified to receive services through AHRC, NYC.

    _ Salary_ : $40,000 -$45,000 per year plus a comprehensive Benefit package. See Benefit information below.

    ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

    • Works with Central intake staff to coordinate referrals and admissions into identified services.
    • Works with individual and family to facilitate enrollment into selected AHRC, NYC services.
    • Schedules and coordinates evaluations needed for OPWDD eligibility determinations.
    • Schedules and coordinates meetings between the individual/family to ensure proper benefit coverage as needed for the identified services.
    • Reviews all intake material for regulatory and eligibility compliance.
    • Work with agency intake staff to ensure enrollment packages are complete and in compliance with OPWDD eligibility standards.
    • Works with Care Coordination staff to obtain any needed documentation
    • Submits to local DDROs all eligibility determination information for services as needed.
    • Maintains tracking system for all intake referrals.
    • Maintains ongoing communication with local DDROs to ensure a positive working relationship and timely completion of intake process.
    • Other duties may be assigned.

    SUPERVISORY RESPONSIBILITIES

    There are no direct supervisory responsibilities connected with this position however, the Placement Specialist may be asked to assist in training employees; planning, assigning, and directing work as is applicable to his or her position.

  • Qualifications

    Qualifications

    • A Bachelors degree in either education, social work or psychology or 4 to 10 years of related experience is required.
    • A strong knowledge of OPWDD eligibility.
    • Do you possess expert computer skills, including Microsoft Office Suite, with particularly strong skills in Excel.
