Description of Duties:

Performs duties as assigned by Case Manager /RN/licensed therapist including baths, oral hygiene, nail care, footcare, skin care, pressure relief, dressing patient, feeding, and transferring patient.

Take accurate vital signs.

Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

Identifies and communicates findings to the Case Manager/RN/licensed therapist

Other duties as assigned.

Qualifications:

Valid/Current MD Certified Nursing Assistant certification

Valid/Current CPR and First Aid Certifications

At least 1 year professional experience as a caregiver preferred

Open availability, able to work various shifts to include weekends

Strong interpersonal skills
