Join the team at Ponessa Behavioral Health as a Care Coordinator for the Substance Use Disorder Outpatient Services Program. This is a part time, fee for service position, with the option to transition into full-time.
Job Description:
The Care Coordination services are comprised of the non-clinical services including but not limited to: Education, Vocation, Employment, Physical health, Emotional/Mental health, Family/Social needs, Living arrangements/housing, Legal status, Basic needs of food, clothing, and utilities, Life skills, Child care, Transportation, Engagement into Medication Assisted Treatment (MAT). These non-clinical services are an effort to assist individuals in their attempt to recover from substance use disorder-related problems. Care coordination of these recovery support services is essential before, during, and after the treatment episode. Linking and coordinating a client to the available support services for the identified non-clinical treatment-related needs before, during, and after the treatment episode increases the probability that a recovery-oriented self-sufficient lifestyle may occur. The ability to identify and match the Client to the non-clinical treatment-related support services prior to the treatment episode, during the treatment episode, and following the treatment episode enhances the foundation of the recovery process for the Client, establishes a collaborative process between the systems working with the Client, and contributes to the evolution of a Recovery-Oriented System of Care (ROSC).
The role of a Care Coordinator is an individual with personal, lived experience of their own recovery. By offering insight into the recovery process, Care Coordinators are able to share a unique perspective with those with similar life experiences. Care Coordinators accept and agree that their experience as a person in recovery from a substance use disorder will be known by their colleagues, individuals served, and others with whom they may share that they have achieved this credential. The Care Coordinator is a role model, mentor, advocate, and motivator. Care Coordinators advocate for the individual, the recovery system, recovery-oriented services and assist to identify and reduce stigma. Care Coordinators advocate to eliminate barriers, increase natural supports and build community. Services provided by the Care Coordinator substantially improve an individual's and family's ability to sustain recovery and wellness.
Staffing qualifications/credentialing/clearance requirements:
Qualifications: Associate's Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services) or another related field. Proficiency in documentation is essential. A valid driver's license, a personal cell phone, and an insured vehicle for transporting clients is required. Staff in recovery from a substance use disorder, must have a minimum of two years of uninterrupted recovery and currently working a personal program of recovery. For Staff in recovery from co-addiction, must be working a personal program of recovery. Must possess the Pennsylvania Certification Board (PCB) Certified Recovery Specialist (CRS) or Certified Family Recovery Specialist (CFRS) credential one-year post hire. These trainings are required to meet the credentials of a CRS or CFRS. Upon certification, the individual must attend 30 hours of training per two years and continue to update their certification every two years. Any individual who does not have an Associate's Degree, CRS, and/or CFRS will be required to actively work on obtaining these degrees and/or certifications.
Clearances: (Prospective employee must have the following clearances before servicing clients.) 1. PA State Child Abuse Clearance (Upon hire, before working with clients, and every year thereafter) 2. Pennsylvania State Criminal Background Check (Upon hire, before working with clients, and every year thereafter) 3. FBI Clearance. It is also required for all employees to complete and provide a certification of completion for Mandated Reporter Training.
