    Job Description

    ** OVERVIEW:**

    McClendon Center is seeking compassionate and motivated individuals to serve as Post-ER Engagement Specialists (PEES). In partnership with AmeriHealth Caritas DC, this position focuses on improving post hospital follow-up care for high-risk Medicaid members following emergency department (ED) visits.

    This is a unique, field-based role located at Washington Hospital Center and Howard University Hospital, where you’ll play a vital role in helping vulnerable individuals access the services they need to improve their health and well-being.

    ** POSITION SUMMARY:**

    As a Post-ER Engagement Specialist (PEES), you will proactively engage members who frequently use emergency room services and may struggle to connect with ongoing medical or behavioral health care. Using the Event Notification System (ENS) provided by AmeriHealth Caritas DC, you will locate members in person at the emergency room waiting rooms, complete historical documentation, and connect the member to appropriate follow-up services to include primary, medical, and/or behavioral health services.

    ** PRINCIPAL ACCOUNTABILITIES:**

    • Review ENS reports to identify high-risk Medicaid members following ED visits.
    • Verify member eligibility using NaviNet.
    • Make at least three substantial outreach attempts per member, in accordance with AmeriHealth Caritas DC guidelines.
    • Contact members to conduct or coordinate behavioral health screenings in collaboration with the Patient Discharge Coordinator Clinical Supervisor.
    • Conduct health and behavioral screenings to determine immediate care needs.
    • Refer members to appropriate services, including:
      • Primary care or specialist post-discharge appointments
      • Intake appointments with Community Support Agencies (CSA)
      • AmeriHealth Caritas DC care management programs
    • Follow up with each member to confirm attendance at scheduled appointments and assist with rescheduling if needed.
    • Collaborate with hospital staff, AmeriHealth Caritas DC, and internal teams to support seamless care coordination.
    • Document all outreach efforts and outcomes thoroughly in the Credible system; documentation must be timely, accurate, and descriptive.
    • Track progress toward Referral/Completion status in accordance with program requirements.

    ** PROGRAM IMPACT:**

    This program reduces non urgent ED visits, promotes access to outpatient health services, and helps members stay engaged in their aftercare plans. As a PEES member, your work directly improves the health and quality of life of vulnerable members of the community.

    ** COMPENSATION:**

    • PEES are compensated a flat rate of $210 per located and engaged member, once all conditions for Referral/Completion have been met as outlined by AmeriHealth Caritas DC.

    Qualifications

    Must pass a background check and be free of any local or federal sanctions or exclusions. Must maintain client/member confidentiality at all times. Must conduct oneself with appropriate decorum so that the sterling reputation of McClendon Center is upheld at all times. Must operate with high ethical and moral standards, being forthright and honest at all times.

    ** MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:**

    • High school diploma or GED required; associate’s or bachelor’s degree in social work, psychology, healthcare, or a related field preferred.
    • Minimum of 1–2 years’ experience in healthcare, behavioral health, case management, outreach, or community support services.
    • Strong communication, engagement, and motivational interviewing skills.
    • Ability to work independently in a hospital setting and exercise sound judgment.
    • Strong skills in data skills, including Excel, is essential.
    • Experience with EHR systems, Credible and NaviNet, is a plus.
    • Knowledge of Medicaid and outpatient health services preferred.
    • Comfortable working with culturally diverse populations while maintaining a respectful and supportive approach.
    • Must be organized, detail-oriented, and capable of documenting activities clearly and on time.

    Hours/Schedule:

    • Contracted hours available (24/7)
