    Overview

    Posterity Health is the Center of Excellence for Male Care. WE focus on male fertility, sexual health and male hormone management. We enable couples having trouble conceiving to proactively assess and treat male factor conditions, in addition, we work with males focusing on Total Health and Wellness. We have assembled a team of fellowship trained Reproductive Urologists who partner with APPs to deliver personalize care to all patients.

    We believe males deserve a specialized care team and an outstanding customer experience. We offer virtual visits, at home diagnostics and in-person consults. If you have your own practice, let’s talk – we are building something special.

    Responsibilities:

    • Evaluate and provide care for our hormone management patients. We mean, really take care of them! We are looking for supportive, inspiring clinicians to attract patients and drive compliance.
    • Evaluate patients, collecting and reviewing medical history and performing an appropriate physical exam in the outpatient setting.
    • Interpret and evaluate patient data as authorized by a physician for the purpose of determining management and treatment of patients. Consult with physician as appropriate.
    • Adjust patient care plan by reviewing and monitoring treatments and therapy plans.
    • Order laboratory, radiological and diagnostic tests that will assist with the medical management; explain the necessity, preparation, nature and anticipates effects of treatments, and diagnostic and therapeutic procedures to the patient.
    • Prescribe and regulate appropriate pharmacologic medicine.
    • Document appropriately and communicate with the appropriate physician in a timely manner as required, including, but not limited to evaluations, treatment plans, progress reports, billing documentation, treatment notes, and other reports as requested.

    Requirements:

    • At least two years experience providing Male Hormone Management
    • Licensure and Certification as a Nurse Practitioner
    • Valid DEA Drug License or ability to provide one prior to start date.
    • Prescriptive Authority or have Provisional Prescriptive Authority
    • Current ACLS, BLS and CPR certification or ability to provide prior to start date.
    • Enjoys working as a team
    • Strong computer skills in an Electronic Medical Record preferred.
