Job description

As a Home Health RN, responsibilities not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy and standards of community health practice. Also required to perform admission processes and case management responsibilities in accordance with policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.

Providing clinical services to patients from birth to over 100 years in their homes. Assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. The admissions process and case management responsibilities are performed in accordance with MGB Healthcare at Home’s policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.

Qualifications

EDUCATION, EXPERIENCE, TRAINING

1. 1 year Traveler Experience
2. 1 year Home Care experience.
3. 1 year Case Management Experience
4. Current licensure in the state of Massachusetts
5. Reliable, independent means of transportation to make home visits.
6. Must be willing to work weekends/holidays/on call in accordance with department need/rotation Responsibilities:
