MENTAL HEALTH THERAPIST- Fond du Lac
MENTAL HEALTH THERAPIST SUMMARY:
The MH Therapist is a member of the Treatment Team. S/he will be responsible for completing Mental Health Assessments and providing therapy to each client assigned to his/her caseload. S/he will provide individual and/or group therapy as determined by patient need. The Program Director will be the immediate Supervisor.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Abide by all clinic policies and procedures as documented in the Personnel manual and in the Policy and Procedure Manual.
Attends all staff meetings unless previously excused by administration or unless absent from work.
Abide by the professional ethics of the individual’s professional discipline and organization.
Will maintain full compliance with state/federal rules and guidelines regarding treatment.
Adhere to all rules of Federal Confidentiality.
Adhere to all requirements established by funding sources for maintaining case records.
Shall be responsible for the accuracy and full compliance of all chart documentation including, but not limited to:
Diagnostic Summaries
Treatment Plans
Treatment Plan Reviews
Progress Notes
Correspondence
Discharge-Transfer Summaries
After-care Plans
Must complete all billing information accurately, and within required time period, as applicable.
Will provide the following services under the direction and supervision of the Program Director:
Assessment of client
Treatment Planning
Case Management
Crisis Intervention
Client Education
Referrals
Case Consultation
Maintain an “audit ready” status with prompt filing of case record documentation.
Continue to attend workshops, seminars, in-services, classes to remain current with the changes, revisions and additions within the profession.
Maintain communication regarding professional treatment of clients with all other departments and shall follow recommendations from other professionals when necessary and applicable.
REQUIRED QUALIFICATIONS:
LPC or LCSW with Master’s degree in psychology, social work, or a related health field. Substance abuse experience/training preferred.
ESSENTIAL KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS:
Must possess demonstrated computer skills and knowledge in MS Office software applications including Word, Excel, Publisher and Power Point.
Must demonstrate the ability to effectively function as a team member.
Must demonstrate the ability to effectively manage multiple tasks concurrently.
Must possess exceptional communication and organizational skills.
Must be able to customarily and regularly exercise discretion and independent judgment in significant matters.
Must be able to relate with empathy and sensitivity to staff, clients and the community.
Has the ability to appropriately interact on the telephone and answer agency questions.
CERTIFICATIONS, LICENSES AND REGISTRATIONS:
Must be licensed as one of the following: LCSW, LPC, or Licensed Psychologist.
