PHYSICIAN (Part time)- Fond du Lac
PHYSICIAN JOB SUMMARY
Responsible for the overall care of all patients of assigned clinic. Performs physical exams, physical assessments, as well as prescribing and managing the methadone treatment plan of each patient. Works directly with Clinic Manager and clinical staff as needed for medical and treatment needs of clients.
ESSENTIAL DUTIES AND RESPONSIBILITIES (Includes the following. Other duties may be assigned)
DocumentationResponsible for ensuring that all medical evaluations, medical histories, and physical examinations have been completed before patient receives initial dose. Also responsible for ongoing treatment of patients including signing medical orders, treatment plans, and documentation required by state and federal laws
Case ManagementResponsible for patient’s case management including chronic health problems, acute self limited problems, health conditions, and the management of medication prescribed by Community Medical Services.
ReferralsEvaluation and referral of psychiatric condition and case management of patients under psychiatric care. Medical evaluations as needed to ensure that all healthcare needs are being met.

QUALIFICATIONS
Must be licensed to practice medicine in the State of Wisconsin
Supports standards of professional nursing practice and promotes principles of recovery
Controlled substance license, drug control license and DEA registration, ASAM/ABAM or certified physician or will need to obtain certification within 24 months.
Specific training and/or experience in the field of substance abuse, required
Must have superior documentation skills and be committed to excellence
Performs other duties as assigned.
