  • Responsibilities

    Are you a Clinical Social Worker who has experience completing psychosocial assessments, developing treatment plans that are congruent with the overall health care plan, providing psychosocial intervention, educating patients/families, working with the community to provide needed services and coordinating discharge planning activities? If so, we have the perfect role for you!

    Your daily responsibilities:

    • You will initiate patient/family intervention based on assessment of service needs.
    • You will initiate community referrals based on identified patient/family needs.
    • You will contact the appropriate Social Services Department to report suspected abuse, neglect or exploitation.
    • You will be responsible for establishing and maintaining a professional rapport with providers, patients/families and internal/external customers.
    • You will serve as a liaison between medical facility and community agencies or facilities for the exchange of clinical and referral information.
    • You will perform other related duties incidental to the work described herein.

    EDUCATION

    - Required: Master of Social Work Degree from a Council or Social Work Education-accredited program.

     

    EXPERIENCE

    - Required: A minimum of one (1) year of clinical social work experience.

    - Preferred: Minimum of two (2) years of clinical social work experience in an acute care setting.

     

    Degrees, Licensure, and/or Certification: Must have current/active licensure in the state of Virginia.

     

    KNOWLEDGE, SKILLS, ABILITIES

    • Able to independently seek out resources and work collaboratively.
    • Ability to establish and maintain effective working relationships.
    • Able to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, and others.
    • Able to record activities, document assessments, plan of care, interventions, evaluation and re-evaluation of patient status.
    • Able to remain focused and organized.
    • Working knowledge of infection control procedures and safety precautions.
