General Responsibilities: Coordinates activities programming for independent and personal care residents; planning, organizing, and implementing specific procedures and programs designed to meet the interests and the physical, mental, and psychosocial well-being of each resident. Provides exceptional customer service in support of Presby's Service from the Heart. Essential Responsibilities 1. Works with the Director of Recreational Therapy and Activities to create a diverse program and outings that assure a blend of educational, physical, spiritual, intellectual and psychosocial programs that meet the individual needs of each resident. 2. Demonstrates familiarity with the individualized care plan for residents and provide support to the resident according to the care plan. Contributes to the care planning process by attending care plan meetings and providing information to the care plan team. 3. Provides all documentation and reports required to assess individual needs and preferences, establish goals, and record outcomes. Keeps all documentation accurate and timely. 4. Keeps residents, staff, and others informed of activities and related information through calendars, newsletters, or other forms of communication. 5. Promotes a home-like environment for residents and maintains neat and clean work areas including supply and storage areas. 6. Maintains current professional development and knowledge, such as through literature, seminars and professional group associations; including sharing with other Presby staff. 7. Supports the department in absence of the Director. 8. Responsible for communicating supply needs necessary for programming. 9. Provides in-services as assigned and support and training of staff and volunteers. Qualifications Experience: One year's previous experience in a health care setting or with a geriatric population preferred. Education/Training: A B.S. degree in recreational therapy or related field and/or activities certification; certification as a therapeutic recreation specialist or activities professional; or two years' experience in a social or recreation program including one year of activities in a health care setting. Skill(s): Proficient reading, writing, grammar, and mathematics skills; Excellent interpersonal relations and communicative skills; proficiency with standard computer programs (ie. Microsoft office suite); knowledge of Pennsylvania Department of Public Welfare personal care/Department of Health regulations relating to activity programming.
