    What You'll Do

    • Deliver longitudinal primary care
    • Manage transitional care post-discharge
    • Reduce hospital readmissions
    • See patients where they live — private homes, assisted living, skilled nursing (~30%)
    • Oversee care across a defined regional footprint
    • Build real patient relationships

    Must-Haves

    • MD or DO
    • Board Certified/Eligible IM or FM
    • Active state license (or willing to obtain)
    • Comfortable with community-based care delivery
    • Eligible to work in the U.S.

    Nice to Have

    • Experience in home-based, ALF, or SNF settings
    • Interest in regional leadership growth
