$5,000 SIGN-ON BONUS FOR FULL-TIME RN Homecare Primary RN - M-F 8a-430p, every 4th weekend and 2 holiday rotation Summary of Position: The Primary Nurse is responsible for direct patient care. Responsibilities: Responsibilities of Position: • Provide competent nursing care to assigned patient through skilled symptom management, emotional support, appropriate teaching, and timely response to patient and family needs, including; • Routine Home Care Visits • End of Life/Continuous Care • Referrals and Admissions • Care Center Assistance • Chart Reviews • Other duties as assigned • Coordinate IDG planning for assigned patients • Participate in the IDG analysis of the patient resulting in the development, implementation, and revision of the IDG Plan of Care. • Teach, supervise, and evaluate the care provided by volunteers, hospice aides, and primary caregivers • Facilitate continuity of care by coordinating admission of home-care patients to inpatient settings (hospital, nursing home), as needed • Explain hospice services and benefits in regards to Medicare, Medicaid, and Private insurance, and know when to use Palliative Care Interventions and ABNs • Obtain and enter orders from Medical Directors and Physicians when deemed appropriate • Order appropriate Durable Medical Equipment (DME) for the patient • Perform competent initial nursing assessments • Work evenings and weekends as needed to meet the needs of the patients and families, and the agency's backup schedule • Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements • Maintain clinical competency for assigned responsibilities and procedures • Other duties as assigned Qualifications: Education and Experience: • Graduation from an approved School of Nursing • Current RN licensure in the State of Ohio • Minimum of two years of experience in Medical/Surgical, Oncology, or community health nursing • Hospice experience preferred Special Skills and Requirements: • Shall have a valid driver’s license • Excellent customer service skills • Detail-oriented with excellent communication, documentation, and organizational skills • Ability to follow instructions • Ability to work independently without supervision • Travel to other sites as needed. Travel may be in inclement weather • Ability to lift 25 lbs of supplies or equipment • Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking • Ability to assist with patient care activities such as ambulation, transfers, and positioning
