    Position: Licensed Master Social Worker (LMSW)

    Rate: $40/hour

    Contract: Annual contract

    Location: Amityville, Suffolk, NY 11701 Job Summary:We are seeking a Licensed Master Social Worker (LMSW) to provide Community Integration Counseling (CIC) services under the NHTD (Nursing Home Transition and Diversion) waiver program. The LMSW will support waiver participants who are experiencing emotional difficulties in adjusting to a significant disability and living in the community. This counseling service aims to assist individuals in managing their altered abilities, revised expectations, and changing roles within their relationships. The LMSW will work directly with the waiver participant to promote community integration and improve their overall well-being.

    Responsibilities:

    1. Provide individualized counseling services to waiver participants under the NHTD waiver program.
    2. Assist participants in managing emotional difficulties associated with their disability and community living.
    3. Conduct initial assessments and develop counseling goals in collaboration with the participant.
    4. Deliver counseling sessions primarily in the provider's office or the participant's home.
    5. Conduct face-to-face sessions with the waiver participant for all CIC sessions.
    6. Offer one-to-one counseling, family counseling, or group counseling sessions as appropriate and with participant consent.
    7. Maintain client confidentiality in accordance with federal and professional standards.

    Requirements:

    1. Must hold a valid license as a Master Social Worker (LMSW) in the state of New York.
    2. Experience working with individuals with disabilities and those transitioning from nursing homes is preferred.
    3. Strong counseling and interpersonal skills.
    4. Ability to assess emotional difficulties and develop appropriate treatment plans.
    5. Knowledge of community integration principles and counseling techniques.
    6. Excellent communication and documentation skills.
    7. Must be reliable, compassionate, and dedicated to promoting the well-being of the waiver participants.
