Job Description:

We are seeking a Licensed Master Social Worker (LMSW) to join our team in providing services to individuals with Traumatic Brain Injuries (TBI) through the TBI Waiver Program. The LMSW will work directly with our ILST clients, assisting them in developing and implementing individualized service plans that promote their independence and community integration.

Hourly Rate: $30-$35 per hour

Responsibilities:

• Conduct assessments to identify client needs and develop individualized service plans in collaboration with the ILST team.

• Provide direct services to clients, including counseling, education, and advocacy, to promote their well-being and social functioning.

• Coordinate with other healthcare professionals, community resources, and service providers to ensure clients receive the necessary support.

• Monitor and evaluate client progress regularly and adjust service plans as needed.

• Maintain accurate and timely documentation of all client interactions and service delivery.

• Participate in ongoing training and professional development opportunities to enhance knowledge and skills.

Qualifications:

• A current New York State LMSW license is required.

• Experience working with individuals with TBI or other neurological disorders is preferred.

• Knowledge of the TBI Waiver Program and related regulations is a plus.

• Excellent communication and interpersonal skills are essential.

• Ability to work independently and as part of a team.
