    Position Summary

    The Private Duty Nurse (Registered Nurse or Licensed Practical Nurse) provides one-on-one, skilled nursing care to clients in their homes under the supervision of a physician and in accordance with the client’s individualized Plan of Care (POC). The nurse ensures clinical excellence while demonstrating compassion, integrity, and faith-driven service that promotes the client’s comfort, independence, and dignity.

    Private Duty Nurses may care for pediatric, adult, or geriatric clients with complex medical conditions, including those requiring ventilator support, tracheostomy care, or other continuous skilled interventions.

    Key Responsibilities

    · Provide direct, individualized nursing care in the client’s home following physician orders and POC guidelines.

    · Perform comprehensive assessments, monitor vital signs, and observe physical, emotional, and behavioral changes.

    · Administer medications and treatments, including IV therapy, wound care, tracheostomy, and ventilator management as appropriate.

    · Maintain accurate, timely documentation of all services provided per agency standards.

    · Collaborate and communicate effectively with the care team, including physicians, therapists, caregivers, and the Director of Nursing.

    · Educate clients and family caregivers on health conditions, safety, and self-care strategies.

    · Adhere to infection prevention, medication administration, and patient safety protocols.

    · Uphold HIPAA confidentiality and professional ethical standards.

    · Respond promptly to changes in client condition and report concerns to the appropriate supervisor or physician.

    · Participate in case conferences, continuing education, and quality assurance initiatives.

    · Deliver all care in alignment with Blessing Hands Home Care’s faith-based mission of compassionate service, dignity, and excellence.

    Qualifications

    · Current, unrestricted RN license in the state of Missouri (Compact license preferred).

    · Minimum of one (1) year of clinical experience, preferably in home health, pediatric, or critical care settings.

    All Applicants Must Also Have:

    · Current CPR certification (AHA or Red Cross).

    · Reliable transportation, valid driver’s license, and current auto insurance.

    · Strong communication, assessment, and organizational skills.

    · Ability to work independently while maintaining professional collaboration with the interdisciplinary team.

    Work Schedule

    PRN: Flexible scheduling based on client needs and nurse availability. Ideal for nurses seeking supplemental or occasional shifts.

    Part-Time: Regularly scheduled hours (typically 20–30 hours per week), with possible evening, night, or weekend shifts as needed.

    Physical Requirements

    · Ability to lift, move, or assist up to 50 lbs as needed.

    · Frequent standing, walking, bending, and assisting with client mobility or positioning.

    · Must be physically capable of performing essential nursing duties and successfully complete a Health Assessment per agency policy.
