About the Role:

Tradition Home Health Care in Brooklyn, NY is looking for a passionate and dedicated Private Duty Travel Nurse to join our growing team. Responsibilities:

Provide one-on-one private duty nursing care to patients in home and travel settings

Administer medications, treatments, and therapies as prescribed by the attending physician

Monitor and document patient health status, vital signs, and progress

Develop and implement individualized patient care plans

Coordinate with physicians, specialists, and family members to ensure continuity of care

Travel with and accompany patients as needed, ensuring safety and comfort during transit

Respond promptly to patient needs and medical emergencies

Requirements:

Active RN or LPN license in good standing (multi-state licensure or compact license a plus)

Previous private duty, home health, or travel nursing experience preferred

Strong clinical assessment and critical thinking skills

Ability to travel with patients locally, domestically, or internationally as required

Excellent communication and interpersonal skills with patients and families

Current BLS/CPR certification required; ACLS preferred

Flexible, adaptable, and comfortable working independently in varied environments
