    GENERAL SUMMARY:

    The Team Assistant [LVN Scheduler] processes and coordinates all required clinical documentation and communication (paper and electronic) to facilitate complete, accurate and timely flow of patient and clinical information. This includes, but is not limited to, patient visits scheduling, MD orders management, assigned workflow processes, and general clerical support to Clinical Supervisor and assigned clinical Team functions. This position is Full-Time with an expectation to work in person at our office in Wilshire Park M-F 8:30am-5:00pm. Learn more about our Company on our website: https://www.prohealthca.com/#~87Z3Z93

    PRINCIPAL DUTIES AND RESPONSIBILITIES (Included but not limited to):

    Facilitates maintaining updating and communicating visit schedules for Team field staff.

    Communicates with patients and outside vendors (lab services) on an on-going basis.

    Updates patient schedules timely and accurately in the EMR system.

    Distributes communications to appropriate staff as directed.

    Maintains assigned tracking systems with accurate/up-to- date information.

    Assists with pre-bill audits as assigned.

    Receives, addresses, triages, and relays messages for assigned Team field staff.

    Assists with weekend patient scheduling and on-call coverage.

    Receives and distributes incoming faxes including, physician orders, lab results, etc.; prepares and sends outgoing faxes.

    Participates in quality management activities as assigned.

    Assists with the organization and preparation of office projects

    Demonstrate necessary skills and knowledge to coordinate work flow.

    Supports Agency’s customer service reputation by promoting and maintaining a responsive, knowledgeable, and accessible approach to all customer contacts.

    Prepares reports accurately and timely.

    Other duties as assigned by the Clinical Supervisor, Director of Professional Services and /or Administrator.

    EDUCATION, EXPERIENCE and LICENSES:

    High school diploma or equivalent and graduate from an accredited Licensed Vocational Nursing Program.

    Minimum of one (1) year as an LVN in the Home Health Care Setting

    Minimum of one (1) experience and above average in computer processing.

    BLS Certified

    SKILLS AND ABILITIES:

    Knowledge of medical terminology

    Excellent Communication skills; ability to be flexible and work well with others.

    Ability to maintain confidential information.

    Ability to prioritize and multitask.

    Ability to work with a flexible schedule

    Excellent written and verbal communication skills

    PHYSICAL REQUIREMENTS:

    Prolonged periods of sitting at a desk and working on a computer.

    Must be able to lift up to 50 pounds at times.

    Ability to drive a motor vehicle for extended periods of time.
