    Clinical Supervisor - $10,000 SIGN-ON BONUS!

    ProHealth Home Care Services LLC, is seeking a Clinical Supervisor to direct, coordinate and evaluate the delivery of home health services to a team of patients ensuring compliance and consistency with agency policies, state and federal home health regulations. This individual will monitor attainment of the objectives of the team activities. The Clinical Supervisor will provide direct clinical supervision and perform in-home supervisory visits with assigned team members.     PRINCIPAL DUTIES AND RESPONSIBILITIES (Included but not limited to):

    Provides direct clinical supervision, performance evaluations and disciplinary action of all assigned staff. Obtains input from the Director of Professional Services (DPCS). Evaluates and ensures that competency requirements are met for all the staff.

    Ensures and performs in-home supervisory visits with assigned team members.

    Monitors staff assignments, caseloads, and team productivity. Promotes efficient utilization of resources to achieve desired clinical outcomes and within episode reimbursement.

    Support the coordination of interdisciplinary care.

    Develops and maintains a system of patient tracking and financial forecasting. Consults with finance to ensure OASIS coding results in appropriate reimbursement for the patient’s clinical condition. Refers and discusses high utilization and problem prone cases with leadership.

    Provides the Administrator and Director of Professional Services with census, staffing, team productivity and other reports as requested. Identifies staffing patterns and needs of the assigned team.

    Facilities appropriate communication and documentation between patient, team members, medical professionals and referral sources.

    Coordination of lab results to all physicians timely.

    Conducts individual caseload reviews and team meetings on a regularly scheduled basis according to the agency’s standards. Case Conferences with staff as needed to meet the needs of challenging patients.

    Participates with the Administrator and Director of Professional Services in interviewing, screening, and selecting candidates for team positions.

    Take On - Call when necessary.

    Acts as a role model and a mentor professionally and clinically for the implementation of the organization’s Mission and Value Statements.

    Works with Quality Staff to ensure accurate and timely documentation completion.

    Implements the staff orientation and development program in conjunction with the Quality.

    Represents the agency on committees/professional organizations and increases community awareness of agency in selected public relations efforts.

    Knowledgeable of State, Federal, local rules and regulations.Title XXII standards and accreditation regulations for the delivery of home health services.

    Performs other duties as assigned.

    EDUCATION and EXPERIENCE:

    Registered Professional Nurse , a graduate of an approved school of professional nursing who is currently licensed as a registered nurse in the state of agency operation.

    Two(2) years experience as a RN with (2) two years in community health/home health program and maintain CPR certification while employed in a clinical role.

    Previous supervisory or administrative experience is a plus.

    Prior use of Electronic Medical Records (EMR) preferred.

    BS in Nursing is preferred.

    Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and in good working order.

    SKILLS and ABILITIES:

    In-depth understanding of Medicare Conditions of Participation and payment mechanisms for home health.

    Strong organizational, planning, problem solving, decision-making and leadership skills required.

    Strong interpersonal skills. Ability to communicate effectively in writing and verbally with people who have varied backgrounds.

    Ability to direct and manage a multi-disciplinary team of home health care clinicians, both professional and para - professional.

    Computer skills and ability to use word processing database, spreadsheet, applications, etc. preferred.

    PHYSICAL REQUIREMENTS:

    Prolonged periods of sitting at a desk and working on a computer.

    Ability to drive a motor vehicle for extended periods of time.

    Must be able to lift up to 15 pounds at times.
