    Job Summary:

    The Clinical Pharmacy Specialist Primary Care will collaborate with primary care providers and staff to improve the health of SJHMG patients. He/she will provide collaborative drug therapy management services, assist medical personnel and patients with drug information and identification, develop and sustain productive patient relationships, make patient needs a primary focus, evaluate and resolve potential drug therapy problems identified through any and all available sources, interview patients to obtain information regarding drug use, drug allergies/sensitivities and document the information in the patient’s medical record or pharmacy information system, confer with medical personnel regarding care and treatment of patient related clinical diagnosis, drug combinations and dosage forms, and other factors that might influence the course of treatment and the activity of the drugs, including drug identification, suggesting changes in drug therapy and/or use as appropriate. Provide direction to pharmacy interns, residents, fellows, pharmacy technicians and other pharmacy personnel.
    The Clinical Pharmacist Specialty Primary Care supports SJHMC Primary Care & SJH Pharmacy Services by managing and overseeing the prescribing, dosing modification, EMR documentation, and surveillance of medication to optimize patient clinical care outcomes and avert the risk of harm.

    The Clinical Pharmacist Specialty is an autonomous highly skilled Pharmacist integrated into the Primary care setting to provide Medical Staff, Clinical staff, Pharmacy Staff, and Patient education regarding the prescribing, monitoring, and optimization of medication therapies and improve patient clinical outcomes and disease state management Policy development, continuous process improvement, continuing education, clinical staff development, Pharmacy intern preceptorship, and supervision of Pharmacy Technician

    • Knowledge of pharmacy law and regulations
    • Excellent communication skills
    • Problem solving skills and decision-making abilities
    • Clinical background including influencing medication prescribing habits
    • Ability to work as a team member

    Minimum Position Qualifications:
    Education:

    • Graduation from an Accredited school of pharmacy.

    Experience:

    • Two (2) years pharmacy experience including clinical activities (whether Acute Care or Ambulatory Setting)

    License / Certification:

    • Active California Pharmacist license
    • ACLS Certification (Within 120 days of hire)
    • BLS Current

    Preferred Position Qualifications:
    Education:

    • Pharm.D. degree

    Experience:

    • Two (2) years experience (Acute Care or Ambulatory Setting)
    • Completion of a PGY1 or PGY2 Residency

    License / Certification:

    • Board Certification
    • CA Licensure as an Advanced Pharmacist Practitioner
