SALARY: $41,000 – $43,000
Sign-on bonus of $1,000
$1,000 INCREASE EVERY SIX MONTHS FOR THE FIRST TWO YEARS IF MEETINGS RESPONSIBILITIES
This position provides intensive in-home support and treatment to families who are participating in Comprehensive Community Services (CCS) to stabilize and strengthen the family unit.
KEY RESPONSIBILITIES:
- Interview and evaluate families to complete the intake process and assess therapeutic needs.
- Participate in the formulation, implementation, and evaluation of Individualized Service Plans.
- Counsel clients in accordance with their treatment plans.
- Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.
- Demonstrate knowledge and skill in addressing family system, mental health, parenting, and alcohol/drug issues through individual, family, and group counseling.
- Conduct home visits per service plans.
- Attend youth/family team meetings.
- Report youth progress to the Families First Project and Kenosha County Department of Human Services and make appropriate recommendations.
- Organize and facilitate structured group, individual and/or family activities to meet the goals of the service plan.
- Attend client court proceedings and provide updates on treatment progress, as needed.
- Provide thorough and accurate updates on progress toward goals and make appropriate recommendations in regular and special staffings.
- Coordinate services and maintain communication with all service providers involved with the clients.
- Complete proper documentation as per program standards including note/billing, monthly summaries and discharge summaries.
- Ability to operate a motor vehicle safely and obey all traffic laws. Must have reliable transportation on a daily basis, a valid driver’s license and automobile insurance.
- Safely transport clients as needed.
- Other duties as assigned.
QUALIFICATIONS & REQUIREMENTS
- Master’s degree in Social Work, Psychology, Criminal Justice, or other related fields.
- Excellent written and oral communication skills
- Valid driver's license and proof of automobile insurance
- Criminal background check and drug test required
