    Job Salary: $17.25 an hour

    This position provides services to youth who have been placed out of their home in corrections, in the ACE Program and in group homes. The Liaison works with the youth and the institution to ensure that an effective plan of care is established to address the current treatment needs so that the youth can be returned home or to a less restrictive placement as quickly as possible.

    Key Responsibilities:

    • Interview and evaluate clients to complete intake process.
    • Analyze client situations, capabilities and problems to determine services required to meet treatment needs of client.
    • Assist Department of Corrections (DOC) and group home facilities in formulating and evaluating treatment plan.
    • Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.
    • Attend all formal OJORS and make appropriate recommendations.
    • Attend weekly ACE staffings/petitions.
    • Develop an aftercare plan, including scheduling reintegration meetings and completing Hillcrest screening.
    • Monitor client progress in DOC and group home programming by conducting visits with clients at various correctional and group home institutions a minimum of one time per month.
    • Coordinate home visits between youth, families, facilities, and DCFS.
    • Maintain monthly contact with the client’s parents/guardians to provide progress updates.
    • Coordinate and facilitate a parent trip to each institution a minimum of one time per month.
    • Report client progress to Kenosha County Department of Human Services and make appropriate recommendations.
    • Formulate units, monthly reports, and discharge summaries per program standards.
    • Ability to operate a motor vehicle safely and obey all traffic laws.  Must have reliable transportation on a daily basis, a valid driver’s license, and automobile insurance.
    • Safely transport clients as needed.
    • Other duties as assigned.

    Education:

    • Associates Degree in Social Work, Psychology, Criminal Justice, or other related fields.
    • Position must keep current on information pertaining to client/family needs through seminars, trainings, media, text and research.

    Knowledge Skills and Abilities:

    • Ability to read, speak and write clearly in English.
    • Ability to exercise good judgment and discretion in establishing work schedule.
    • Must be able to work cooperatively and collaboratively with the Department of Human Services.
    • Must establish and maintain productive and professional relationships with clients, colleagues, the school district, and other community agencies.
    • Must maintain client confidentiality.
    • Ability to prioritize and make appropriate decisions.
    • Ability to work independently and effectively manage time.
    • Must possess strong oral and written communication skills.
    • Ability to provide culturally, ethnically, and gender-sensitive services.
    • Ability to follow directions and implement recommendations.
    • Possess creativity in identifying problems and solutions.
    • Ability to self-motivate and remain on task.
    • Knowledge and compliance with policies and procedures as detailed in the personnel policy and procedure manual.
